Medical ketamine treatments
In the Netherlands, ketamine is now used in various ways within medical care. The most well-known variant is the intravenous (IV) ketamine infusion, which is mainly used in pain clinics for chronic pain syndromes and sometimes for therapy-resistant depression. In addition, there is the nasal spray containing esketamine (Spravato®), a registered drug that may be prescribed under strict conditions for severe depression when other treatments do not produce sufficient results. Both applications fall under mainstream medicine, are conducted in a controlled clinical setting, and require medical monitoring because of possible side effects such as blood pressure rise, dissociation and nausea. Oral esketamine is also being experimented with in research settings, but this form is not yet included in the basic package and falls under so-called "promising care".
Ketamine analogue therapy
In addition to the strict medical uses of ketamine, there is growing interest in a therapeutically assisted oral ketamine treatment with substances that are pharmacologically very similar to ketamine but are not covered by the Medicines Act. These analogues activate the same receptors in the brain (especially the NMDA and AMPA receptors) and, like ketamine, can temporarily loosen fixed thinking and feeling structures. This provides psychological space for processing, insight and behavioural change. Whereas intravenous treatment often feels technical and clinical, an oral variant can actually be more integrated within a holistic therapy process, which emphasises preparation, guidance during the session and integration of the experience afterwards.
When ketamine via hospital?
If a person is eligible for ketamine treatment through regular care, it is wise to follow the medical route. In that case, treatment takes place under the supervision of a doctor, with medical monitoring and the care is (partially) reimbursed. So always try to find out first through your GP or psychiatrist whether you fall under the indications for reimbursed treatments. However, if you do not qualify for these medical pathways, or if there is a need for a more therapeutic and experiential framework, a supervised session with a legal ketamine analogue can be a valuable alternative.
